General Health Services

School health services support student wellness, safety, and academic success. Services may include first aid, illness assessment, medication administration, chronic condition management, health screenings, and coordination with families and healthcare providers.

Is there a school nurse on campus?

Yes. A licensed nurse or trained health staff member is available to assess student needs, provide care, and respond to emergencies during school hours.

Health Services Consent (SB 12 Requirement)

Why do I need to sign a Health Services Consent form?
Under Texas Senate Bill 12, parents/guardians are asked to acknowledge consent for their child to receive certain services from school health staff. This supports transparency and ensures compliance with state requirements.

How often do I need to complete the Health Services Consent?

The Health Services consent will need to be signed each school year for each student.

What is the difference between Health-Related Services and Health Care Services?

Health-Related Services:
These services support student safety and access to learning, including:

First aid and injury assessment beyond routine care
Monitoring of chronic health conditions (such as asthma, diabetes, etc.)
State-mandated screenings (vision, hearing, and scoliosis)

Health Care Services (require additional authorization):
These services involve more direct medical care and require appropriate documentation:

Administration of medication with a completed written permission form from the parent/guardian and, when necessary, a physician
Medical procedures prescribed by a physician and accompanied by a signed physician’s order provided by the parent/guardian

What happens if I don’t sign the consent?
If the form is not signed, consent will be presumed unless a parent/guardian submits a signed consent.

What immunizations are required for school attendance?
Texas law requires students to be fully immunized according to guidelines from the Texas Department of State Health Services. This information can be found at the following links:

What are the requirements for 7th grade entry?
Students entering 7th grade must have:

Tdap (tetanus, diphtheria, pertussis booster)
Meningococcal (MCV4) vaccine
Varicella (Chickenpox)-at least two doses

What if my child has an exemption?
Exemption affidavits must be:

Submitted to the school
Notarized with the original document provided to the school
Renewed every 2 years, as required by the state
Current and in possession of the campus nurse in order for the student to attend school

Medications at School

Can my child take medication during the school day?
Yes, but all medications (prescription and over-the-counter) must follow school policies.

What is required for medication to be given at school?

Written authorization from the parent/guardian
Medication in the original, properly labeled container
Physician order, when applicable

What about self-carry medications like inhalers or EpiPens?
Even if the medication is carried by the student and not stored in the clinic, it must still have:

Appropriate physician documentation
Required school forms on file

Please reach out to your campus nurse for the appropriate medical forms needed for your student to be able to self-carry emergency medications.

Can I send my student’s medication to school with the student?

No. Any medications that are to be given during school hours will need to be appropriately checked in with the nurse by a parent/guardian and have the appropriate forms completed.

Chronic Conditions & Medical Plans

My child has a chronic condition (asthma, diabetes, severe allergies, etc). What do I need to provide?
Parents must submit prior to the start of each school year:

Physician-signed orders
Individualized action plans (e.g., asthma action plan, diabetes management plan, allergy action plan)

Do these forms need to be updated every year?
Yes. New physician orders and action plans are required EVERY school year to ensure safe and current care. Please reach out to your campus nurse for the appropriate forms.

Illness & When to Stay Home

When should I keep my child home from school?

Keep your child home if they have:

Fever (100°F or higher) within the past 24 hours
Vomiting or diarrhea
Symptoms of a contagious illness

Students should be symptom-free for 24 hours without medication before returning to school.

Lice

What are the district’s expectations when it comes to lice?

Head lice are a common concern among school-aged children and are not a sign of poor hygiene. The district follows current public health guidance to manage lice in a way that supports student health while minimizing unnecessary missed instructional time.

Students found to have live lice may be sent home at the end of the school day for treatment. In some cases, a parent/guardian may be asked to pick up the student sooner, depending on the situation.
Treatment should begin before the student returns to school. This typically includes the use of an appropriate lice treatment product and removal of live lice.
Students are generally allowed to return to school after treatment has been initiated. The presence of nits (eggs) alone does not exclude a student from school.
The school nurse may recheck the student upon return and provide guidance if additional treatment is needed.
Parents/guardians are encouraged to check household members and wash or clean personal items (such as bedding, hats, and brushes) as part of treatment.

The district emphasizes education, early detection, and effective treatment while avoiding unnecessary exclusion from school.

Health Screenings

Are students screened for vision, hearing and scoliosis?
Yes. Willis ISD Health Services, under the guidelines of the Texas Department of State Health Services, provides a screening program for vision and hearing for all students in pre-kindergarten, kindergarten, 1st, 3rd, 5th, and 7th grades and students new to Texas.

Spinal screening is conducted for girls in 5th grade and 7th grade and boys in 8th grade. This screening is required by the Texas Department of State Health Services. If you prefer to have this screening done by your family physician, please send the results to the school nurse.

Emergency Information

How often do I need to update our contact information?

Accurate and current parent/guardian contact information ensures:

Quick communication in case of illness or injury
Timely response during emergencies

Changes in address or phone number should be updated ASAP to ensure ease of communication. Please notify the school immediately of any changes.

When should I expect a phone call from the nurse?

Parents/guardians can expect to be contacted by the school nurse when a student has a health concern that may require follow-up, monitoring, or dismissal from school.

This may include situations such as:

Fever or symptoms of illness that may require the student to go home
Injury beyond minor first aid
Head injuries
Ongoing or concerning symptoms (vomiting, difficulty breathing, severe pain, etc.)
Health issues related to a known medical condition (such as asthma or diabetes) that require attention beyond routine care
When clarification or additional information is needed regarding your child’s health history or care plan

For minor issues (such as small scrapes, mild headaches, or routine care), a phone call may not be made, but care will still be provided

If a parent/guardian cannot be reached, the nurse will contact individuals listed on the student’s emergency contact list.

If you ever have questions about your child’s visit to the clinic, you are encouraged to contact the school nurse directly.
