General Health Services
Yes. A licensed nurse or trained health staff member is available to assess student needs, provide care, and respond to emergencies during school hours.
Health Services Consent (SB 12 Requirement)
Under Texas Senate Bill 12, parents/guardians are asked to acknowledge consent for their child to receive certain services from school health staff. This supports transparency and ensures compliance with state requirements.
These services support student safety and access to learning, including:
-
First aid and injury assessment beyond routine care
-
Monitoring of chronic health conditions (such as asthma, diabetes, etc.)
-
State-mandated screenings (vision, hearing, and scoliosis)
These services involve more direct medical care and require appropriate documentation:
-
Administration of medication with a completed written permission form from the parent/guardian and, when necessary, a physician
-
Medical procedures prescribed by a physician and accompanied by a signed physician’s order provided by the parent/guardian
If the form is not signed, consent will be presumed unless a parent/guardian submits a signed consent.
Immunizations
What immunizations are required for school attendance?
Texas law requires students to be fully immunized according to guidelines from the Texas Department of State Health Services. This information can be found at the following links:
2025-2026 K-12 Immunization Requirements (English and Spanish)
2025-2026 Minimum Requirements for Childcare and PreK
What are the requirements for 7th grade entry?
Students entering 7th grade must have:
- Tdap (tetanus, diphtheria, pertussis booster)
- Meningococcal (MCV4) vaccine
- Varicella (Chickenpox)-at least two doses
Additional information on 7th grade immunization requirements for Willis ISD can be found here:
7th grade immunization requirements
What if my child has an exemption?
Exemption affidavits must be:
- Submitted to the school
- Notarized with the original document provided to the school
- Renewed every 2 years, as required by the state
- Current and in possession of the campus nurse in order for the student to attend school
For access to and instructions on how to obtain an Immunization Exemption, please visit the Texas Department of State Health Services at the website below:
Immunization Exemption Information
Medications at School
Yes, but all medications (prescription and over-the-counter) must follow school policies.
-
Written authorization from the parent/guardian
-
Medication in the original, properly labeled container
-
Physician order, when applicable
Even if the medication is carried by the student and not stored in the clinic, it must still have:
-
Appropriate physician documentation
-
Required school forms on file
Chronic Conditions & Medical Plans
Parents must submit prior to the start of each school year:
-
Physician-signed orders
-
Individualized action plans (e.g., asthma action plan, diabetes management plan, allergy action plan)
Yes. New physician orders and action plans are required EVERY school year to ensure safe and current care. Please reach out to your campus nurse for the appropriate forms.
Food Allergies & Diet Modifications
A completed and signed diet modification form from a physician must be on file. This allows the school to safely accommodate dietary needs.
Illness & When to Stay Home
-
Fever (100°F or higher) within the past 24 hours
-
Vomiting or diarrhea
-
Symptoms of a contagious illness
Lice
-
Students found to have live lice may be sent home at the end of the school day for treatment. In some cases, a parent/guardian may be asked to pick up the student sooner, depending on the situation.
-
Treatment should begin before the student returns to school. This typically includes the use of an appropriate lice treatment product and removal of live lice.
-
Students are generally allowed to return to school after treatment has been initiated. The presence of nits (eggs) alone does not exclude a student from school.
-
The school nurse may recheck the student upon return and provide guidance if additional treatment is needed.
-
Parents/guardians are encouraged to check household members and wash or clean personal items (such as bedding, hats, and brushes) as part of treatment.
Health Screenings
Yes. Willis ISD Health Services, under the guidelines of the Texas Department of State Health Services, provides a screening program for vision and hearing for all students in pre-kindergarten, kindergarten, 1st, 3rd, 5th, and 7th grades and students new to Texas.
Emergency Information

-
Quick communication in case of illness or injury
-
Timely response during emergencies
-
Fever or symptoms of illness that may require the student to go home
-
Injury beyond minor first aid
-
Head injuries
-

Reporting Allegations of Bullying
Willis ISD prohibits bullying, including cyberbullying:
- On school property,
- At school-sponsored or school-related activities on or off school property, or
- In any vehicle being used for transporting students to or from school or a school-sponsored or school-related activity.
Bullying may include physical conduct or verbal or written expression, including electronic expression, that was delivered to school property or to the site of a school-sponsored or school-related activity, or off school property or outside of a school-sponsored or school-related activity, if the cyberbullying interferes with a student’s educational opportunities or substantially disrupts the orderly operation of a classroom, school, or school-sponsored or school-related activity.
Bullying is not tolerated by the District, and any student or parent of a student who believes that the student or another student has experienced bullying or that a student has engaged in bullying is encouraged to immediately report the incident. Retaliation against anyone involved in the reporting process is a violation of District policy and is prohibited.
Students or parents may report an alleged incident of bullying, orally or in writing, to a teacher, school counselor, principal, or other District employee. Students or parents may obtain an Incident Report Form at the links below. A student may report the incident anonymously.
K12 Anonymous Alerts
Willis ISD has an anonymous alert reporting system called K12 Anonymous Alerts. Anonymous Alerts® allows students or parents to quickly, easily, and anonymously report urgent information to school officials across all of our school campuses. The mobile app and web-based reporting system provides secure one-way or two-way encrypted messages. All reports remain completely anonymous by default, or you can ask administrators to contact you.
Visit the Willis ISD Anonymous Alerts Page
You can also download the app in your device's app store. The app is a red icon and the Activation Code is "WillisISD."
When an allegation of bullying is reported, the principal or designee shall notify a parent of the alleged victim and a parent of the student alleged to have engaged in the conduct. The principal or designee shall conduct an appropriate investigation based on the allegations in the report. The principal or designee shall promptly take interim action calculated to prevent bullying during the course of an investigation, if appropriate. We encourage you to communicate with your designated campus administrator during this time. Learn more about the district's bullying policy.
Once your report form is filled out completely, submit it by hand delivery, fax or U.S. mail to the appropriate campus administrator. All complaints will be investigated in accordance with FII(LEGAL) and FII(LOCAL) or any exceptions outlined therein.
