• Resignation Form

    If you are a current employee that needs to put in a resignation please click the link above. You will need to print this form and fill it out. After you have completed the form you will need to log into your TEAMS Employee Service Center. Once you are logged into your service center you will chose the tab My Employment Records and then select My Employee Resignation. You will need to upload this form in the My Employee Resignation page in order to submit your resignation.

    Click the link above if you would like to request your records. Please print this form and fill it out and return it to the Human Resources Department. This form can be mailed, faxed, or scanned and emailed to the HR Dept.
