Instructions for Submitting a Resignation
Please click the link above to view detailed instructions on how to submit a resignation from your Employee Service Center.

Resignation Form
If you are a current employee that needs to put in a resignation please click the link above. You will need to print this form and fill it out. After you have completed the form you will need to log into your TEAMS Employee Service Center. Once you are logged into your service center you will chose the tab My Employment Records and then select My Employee Resignation. You will need to upload this form in the My Employee Resignation page in order to submit your resignation.
